Our service is all about increasing our participant independence, so you can achieve your goals and aspirations.
Our support workers can assist you with tasks you cannot do yourself. You may only need this support for a short time to assist you with daily tasks such as showering or dressing or you may need regular support on a long-term basis so you can be as independent as possible in your daily life.

We are specialised in planning and implementing 24 /7 care for participants who are discharging from hospital care. We achieve that by 1:1 consultation with Social Workers, OT, Support Coordinator, and family. In our visit we focus on 3 things
